#Autism, RAD and Puberty: Managing the issues

We met with Dr. Patti this evening about the whole boundaries issue thing with Gavin. Gavin’s at that age where things are changing and hormones are raging. Factor in his lack of impulse control and sprinkle in a few pinches of RAD, you have a recipe for problems.

The mission tonight was to come up with a clear set of boundaries, guidelines and consequences.

The conversation was very uncomfortable and included words like erections and masterbation. This was very awkward but Dr. Patti was a huge help and we survived. I don’t know how much, if anything, Gavin retained but he handled himself well and didn’t get angry or stress out.

We had to reiterate the house rules and establish a few new ones.

Here’s the long and short of it. Gavin cannot have unsupervised access to his brothers. Because of his lack of impulse control, raging hormones and past history, we have to make sure that everyone is same. These rules aren’t anything new but we needed to refocus on this and reenforce it with Gavin. Any violation of these rules will result in swift action.

We are working with Dr. Patti to create consequences for violations and ways in which to implement them.

This is a really challenging time and the combination of Autism and RAD make it more so. Gavin basically functions on very basic, primal instinct and that makes him a handful on the best of days.

Throw in puberty and you have an explosive combination.

In most cases, it’s very much like Of Mice and Men. There’s not necessarily any ill intent behind his actions but those actions can still be damaging to those around him. On the flip-side, because of some of the other mental health issues, ill intent is a possibility as well.

I’m not afraid to say that I’m really nervous about this new leg of our journey. We love Gavin and want us all to be a family, living together and moving forward. At the same time, we have to ensure everyone’s safety and wellbeing.

How do you folks deal with this time of life in your kids on the Autism spectrum? This is an uncomfortable subject but one that obviously impacts a great many of us.

Manic and Psychotic but otherwise okay

Gavin had a pretty good day today, especially considering he’s manic and psychotic. Aside from his incessant talking about totally random and often incoherent thoughts, he’s doing really well.

We’ve not had any major issues this weekend.

There are things going on that seem to be directed at Lizze.  Gavin seems to be testing the boundaries and pushing the envelope a bit, at least as far as Lizze is concerned.

I have been noticing that Gavin seems to be struggling with more and more areas of his life.

He’s having a harder time putting his thoughts together and answering even the most basic of questions. It breaks my heart and I wish we could figure out what is going on.

For right now, I want to just celebrate the fact Gavin is doing pretty well, all things considered.

Gavin’s not feeling right

I’m a bit concerned about Gavin this morning because he says he doesn’t feel right.  Actually he says,  “Daddy, I feel funny”

It’s never a good sign when Gavin says something like that because he almost never complains to us about something like this.  It makes me wonder if we are dealing with either seizures or a pending autonomic crisis.

On the other hand, he could just be feeling dizzy or something all together benign.

There’s no way to know for sure until something either happens or doesn’t happen.  I’m praying that it doesn’t happen. As I await what today brings, I’m a bit anxious and full of worry because the last thing in the world I want is to see my son in the hospital again today.

Have you ever experienced a manic child?

OMG……. That’s about the only way to sum up Gavin today. He is all over the place and extremely manic. Obviously the meds aren’t working yet. It’ll take a little while for them to take effect, that is if they’re even going to.

If I were to describe Gavin being manic, I would say that he’s scattered.

By that I mean while talking, he’s bouncing from one subject to the next. It’s like one giant, never ending run on sentence. It only makes sense to him because his thoughts are so scattered. He also talk a mile a minute and is very, very difficult to understand.

As far as physically goes, he’s walking into things, tripping over his own feet and possesses even less impulse control than usual.

He can also experience rapid changes of emotions.

Don’t get me wrong, he’s not hurting anyone or anything like that. He is however, exhausting and difficult to manage, simply because he’s all over the place. Quite frankly, it’s more annoying than anything else, at least at this point.

For me personally, the incessant talking is what wears me on me. It’s endless and it makes my head hurt because it’s almost nonsensical. I love Gavin, I truly do but OMG……….

We haven’t had the pleasure of dealing with him being this manic in quite some time.

It was bound to happen and we knew it was only a matter of time before his meds would need adjusted once again. Adjusting his meds is more of an experiment because we don’t know how he’s going to react to any changes.

It literally took us years to find a good medication combination that worked well for him without exacerbating his other symptoms.

With a child as complex as Gavin, it truly is a fine balance and requires a great deal of love and patiences.

I would love to hear your experience with bipolar disorder and how you cope with a manic child.

Is the problem behavioral or medical?

I owe you all the rest of the story from our appointment with Dr. Patti on Tuesday evening. We know now that Gavin is in fact both psychotic and manic and we began addressing that tonight with a change in medication.

What I didn’t have time to share with you all yesterday has to do with bladder control issues we’ve been dealing with.

Our appointment in Cleveland that has been rescheduled for the 26th of June, was to rule out any physiological or structural problems that could be causing this to happen.

However, Dr. Patti reminded me last night that this could very well be a behavioral issue.  It’s something commonly seen in kids with Reactive Attachment Disorder and bipolar as well.

I had actually completely forgotten about that.

For a very long time, we had issues with Gavin relieving himself in very inappropriate places, like the floor register in his bedroom, just as an example.

I’ll leave it at that but what Dr. Patti said makes a lot of sense.

However, it’s also possible that this is an autonomic issue as well.  Gavin’s neurologist at the Cleveland Clinic wants to rule out physical problem with his bladder before we explore the autonomic side.

Once again, we have at least 3 possible causes to the bladder control issues Gavin has be experiencing.

Personally, I feel that it’s either behavioral (which he has a documented history of) or autonomic.  Perhaps it’s a combination of the two.  Perhaps it’s something all together different.

Who the hell knows?

This is a perfect example of what makes Gavin so complex.

On the surface, it could be one of three things or some variation or combination as well.

With the history of behavioral issues surrounding urinating in places other than the potty, it could very easily be behavioral. However, with all the autonomic dysfunction and neurological regression, this could be totally outside of his control.

The only thing we can rule out for sure is a structural problem.

Once again the water surrounding Gavin’s condition is made muddy by the many complex pieces that make up who he is.

Frustrating and daunting are the words that come to mind when I think a out how we are going to address this.

The waiting game

We arrived at Gavin’s 3:05pm appointment with his psychiatrist and apparently he’s running 1 hour behind.

Gavin, Lizze and myself did some running around and now only have about a 15 minute wait left.

It’s actually been kinda nice because the three of us never get to spend time together like this.  Gavin’s been having another awesome day and I’m soooooo proud of him.

God willing, we can keep him at an even keel, especially if we have to adjust his medications.

We’ve had a few bumps in the road but with a few minor adjustments, he’s doing really well.  I’ll let you know what happens.  :-)

We have an emergency appointment with Gavin’s psychiatrist

While I was originally really stressed out and worried about rescheduling Gavin’s day at the Cleveland Clinic, it’s actually worked out for the best.

I have the appointments rescheduled already and this time there’s only an hour between each one.

image

This will work loads better for Gavin and it’s only 2 weeks out.  This is a really good thing and I feel much better now that it’s all figured out.

Later on I’ll share what happened at our wraparound meeting this morning but right now we have more pressing matters.

Gavin has an emergency appointment with Dr. Reynolds and this is really important appointment because it appears that Gavin is inside of a psychotic episode and we need to address his meds ASAP before this gets any worse.

Historically, when this has happened, it goes downhill rapidly and he ends up in the psych unit at Akron Children’s Hospital.

I’m so desperately praying that A) we’re wrong, which I don’t think that we are, or B) we can correct what needs to be corrected and pull Gavin back into reality.

This is a topic that is very difficult to understand unless you have experience with it.  Please rest assured that we are doing everything that can possibly be done. This is one of the reasons we were trying to get Gavin into residential treatment.

With Gavin, reality and fantasy has always been a very, very fine line.  At this point, that line is at minimum, blurred, at worst, broken.

Please keep Gavin in your thoughts and prayers today.
Thanks
